By registering and using COVID-19 WATCH, your phone's location information (location, route, and route) will be collected. If you choose to complete the COVID-19 WATCH self-assessment survey, you will be asked to provide your age, gender, and address along with information on current health symptoms (such as fever, dry cough, sore throat, shortness of breath, headache and nausea) and health conditions (such as chronic respiratory, heart, kidney, liver, or neurological diseases, immunosuppression). The self-assessment survey requires contact information, such as name, address, or phone number.


Information we collect from your use of the services
